NEW STUDENT REGISTRATION
Please click on link at the top of this page to download New Student Registration Form. Registration forms for new students are also available in the vestibule of the Church.
They are in a three-drawer plastic box labeled Sacred Heart Religious Education Program.
A copy of the student's baptismal certificate is also required at the time of registration.
Please note: One full year of instruction is required before students prepare for the sacraments of First Reconciliation and First Eucharist in second grade.
One full year of instruction immediately prior to the eighth grade year in preparation for Confirmation.
Please return these forms ASAP. Thank you.

AUTOMATIC REREGISTRATION (For students currently enrolled in Sacred Heart Religious Education Program)
Every student already enrolled in our program in will be automatically moved into the next grade level at same status (Track I, Track II) for the upcoming year.
To change a student's status for the upcoming year, a Student Status Change Form must be submitted to the Religious Education Office.
Forms may be downloaded by clicking on the link at the top of this page. These forms are also available from the three drawer box in the back of the Church.
These forms are also used to indicate a change of student name, address, telephone, emergency phone, current parent/guardian, medical status, learning/physical disabilities etc.
Please remit Student Change of Status Forms to the Religious Education Office as soon as possible! Thank you.
Please note: Enrollment in Track II is subject to availabililty.
(Third grade students in a second grade class for religious education will be moved to fourth grade religious education for the upcoming year. A Student Status Change Form does not need to be used.)
